Please Note:
When filling in an on-screen application, if you see a yellow bar that says, Enable Editing:
you must click on it to continue. Once you've completed filling in your form, save it by clicking on File, then Save As. Name the file and attach to an email to: jac_thompson@hotmail.com
When filling out an on-screen form, if you see "Enable Editing" at the top of your downloaded from, you must click it before you can continue.
Once completed, in your toolbar save your form by clicking File, then Save Page As. Name it and attach it to an email. Send to: jac_thompson@hotmail.com
To fill out an application by hand, go to your toolbar, click File and then click Print.
